Comprehensive Support for those with Chronic Illnesses

A program designed to provide holistic support to those in need, ensuring no one has to face chronic illness alone, with a dedicated focus on uplifting and empowering BIPOC community members who face unique health and systemic challenges.

Financial Assistance:

Financial assistance is provided based on income and
other criteria to cover the costs of lifesaving treatments. ​

Program ensures equitable support for those in dire need.

Must have a referral from your doctor or healthcare caseworker.
​​​​ 2. Enrollment Assistance:

We help participants enroll in affordable healthcare plans, including Medi-Cal, Covered California, and other subsidized
options.
​​​​ 3. Prescription Medications Support:

Financial Assistance for co-payment costs for necessary
medications.

Access to prescription assistance programs.

Must have a referral from your doctor or healthcare caseworker.
​ 4. Healthcare Navigation:

Help navigating the healthcare system.

Guidance to find and utilize suitable insurance plans.

Help reducing the financial burden of healthcare.
PROVIDERS
As a medical provider, you give patients a sense of security through
treatment plans. When personal finances threaten those plans,
patients may feel hopeless. Let California Benefits Support Center’s
Healthcare Access Program help by covering their co-payments,
co-insurance and deductibles for treatments you recommend!

How to Apply:

Participants may call our office or complete the referral/registration form below.

Caseworkers and agents can refer clients by calling the office or completing the referral/registration form below.

Initial contact will involve a screening process to determine eligibility.

Program Qualifications:

Participants must have a chronic illness

Must meet income requirements at 250% of the Federal Poverty Level

Must reside in the South Bay area of Los Angeles County
-
​We serve all community members, targeting low income BIPOC community members (This includes Black, Indigenous, and people of color)
